Incident Title
Chemical Safety During Lab Move Prevents Injury
Incident Type
Chemical Incident
Incident Summary
During a recent laboratory move, a potential chemical incident was narrowly avoided due to strict adherence to safety protocols. While unpacking a box of chemicals, a bottle was accidentally punctured when the user’s thumb broke through the container. Fortunately, the individual was wearing the appropriate personal protective equipment (PPE), which prevented any injuries or chemical exposure.
Upon closer inspection, it was discovered that the chemical was expired and the container was in poor condition. Despite conducting annual inventory reviews to remove expired and unused chemicals, this item had unfortunately been overlooked and remained forgotten in a cabinet by laboratory staff, missing the most recent inventory check.
Corrective Actions / Reminders
This incident underscores the critical need for regular evaluations of chemical inventories, especially during lab relocations, shop cleanouts, or routine audits. Laboratories and facilities shops should take every opportunity to reassess the condition of chemicals and determine if they are still necessary or safe to keep. The experience serves as a timely reminder of the importance of proactive chemical inventory management to prevent potential accidents and ensure the safety of all personnel.
